PRIOR TO YOUR FIRST VISIT
- Download and complete the Intake Forms. Fax or email the Clinic a copy of the completed forms AND bring them with you. This allows Dr. Baile time to look at them before you arrive.
- Ask your medical doctor for copies of any recent laboratory or other test results and bring them with you.
- Bring all medications, supplements and over-the-counter “stuff” that you are taking.
STANDARD INITIAL VISIT
This visit is for those with a long-standing &/or complicated problem and normally takes about an hour. I will take a comprehensive, detailed medical history. I will perform an appropriate physical exam and will review any recent available laboratory results. Medications and supplements will be assessed. Using my many years of training and my extensive clinical experience, I will then explain your personalized treatment options and make my recommendations. You will be asked to participate in deciding on the final treatment plan. You may also be asked to do things like complete a 7 day diet diary or monitor and record the exact timing and severity of certain symptoms. The results of these requests will be assessed at a follow-up visit in 3 or 4 weeks.
SHORT INITIAL VISIT
This visit is for those who have a simple, &/or recent problem. It will be about 30 minutes. I will still take a detailed history about the complaint but not as in-depth a history of all your medical conditions. I will analyze any test reports that you bring and assess the medications, supplements, herbs, etc. that you are taking. I will do an appropriate, modified physical examination and then present a personalized treatment plan for your condition. Follow-up will be in 1-3 weeks as appropriate for your condition.
FOLLOW UP VISITS
These usually occur about a month after the initial visit. They range from 15 minutes to an hour depending how you are feeling, how many questions you have, tests that need to be done, etc. Typically I will see a patient for an initial visit, then a follow-up visit and then not need to see him/her for another 3 or 4 months.
MY TREATMENT PLAN PHILOSOPHY
I strongly believe that the patient must be involved in treatment decisions. It is your body. You decide what you will do with it. There is no point in me producing a wonderful treatment plan if you are not willing to do what I suggest. You will not get better that way. Instead I prefer to determine what changes you are willing to make; what supplements you can afford to purchase, and want to take; how fast you want to heal; and what your priorities are. Working together we can design a treatment plan that you will actually follow and that way we can optimize your healing.
